Melodie is a 26 year old woman who was injured in a car accident about six months ago. Although she experiences a small amount of ataxia, she is physically well-recovered. However, she is left with a number of cognitive deficits, which prevent her from living alone

The COPM with an Alternate Respondent

Mr. Edwards is a 68-year old man with Alzheimer’s disease. He has been admitted for an assessment after an acute episode of behavioural outbursts and unsafe conduct. He lives with his wife, who is 69 years old, and in good health, although very worried and upset about the possibility of bringing her husband back home

Using the COPM with Children

Michael is an 8-year old boy, who has been referred to OT by his teacher, because he has difficulty with math, printing, and following classroom routines. He is often distracted, and therefore seems out of step with the class and with his classmates. He exhibits clumsiness in his daily activities, struggles with Physical Education classes, and tends to be isolated on the playground
